Program Registration
To register your community as a screening program site, you need to complete and submit two forms.
Screening Program Coordinator Registration. Use this form to “officially” register your community in the 2008 program. You must submit your registration no later than March 15.
Screening Program Date and Location. Use this form to report the date and location you’ve scheduled for your program. After you submit this form, the College will send you by UPS ground delivery a screening program manual, along with a kit of educational materials including participant registration forms, posters, flyers and brochures. Although a screening can be held at any time throughout the year, all screening program dates and locations must be reported by April 30 so that materials can be printed and shipped or reserved for you. Please only submit confirmed dates and locations because these will be posted on the ACAAI Web site.
